Top of the foot pain has very few structures involved, but stress fractures, tendonitis and nerve entrapment / numbness are the top 3 in this area.

The ‘sole’ or bottom surface of the foot is comprised of the heel, arch and ball of the foot. This area of the foot comes in contact with the ground and consist of the most common pressure points causing foot pain. Heel spurs, plantar fasciitis, and arch pain all begin and end with poor arch support, increased activity and elongation / strain of the plantar facial ligament. Other problems like metatarsalgia and neuromas are localized on the ball of the foot only.

Treatments vary from conservative care like ice, anti-inflammatories, rest, elevation, physical therapy, supportive shoes and orthotics (see video linked below) to more aggressive conservative therapy like cortisone injections, casting, cam walkers, crutches, and oral / topical pain medications. Surgery is rare, but effective for neuroma removals.
